Enrolment Checklist

NATIONALLY RECOGNISED TRAINING

CHC43515 Certificate IV in Mental Health Peer Work

We have created this checklist to help you complete your enrolment successfully. Please ensure you have all necessary documents ready and have completed all relevant forms before submitting your enrolment to us. This ensures we can process your enrolment efficiently.

You will need the following documents to submit this application. You will be prompted on where to upload the documents in the next steps.

Mental Health Coordinating Council is a registered training organisation RTO 91296.

 

What you need TO DO

Your Unique Student Identifier (USI)

A Unique Student Identifier (USI) is required to enrol in this course. Your USI is a 10 character identifier made up of numbers and letters, required for all students undertaking nationally recognised training.

If you have undertaken any registered training since 2015, you may already have a USI.

Credit Transfer and Recognition of Prior Learning

Credit Transfer and Recognition of Prior Learning are two ways you can gain credit for your previous study or work experience when enrolling in a vocational education and training (VET) course.

Credit Transfer & RPL Form

If you are applying for Credit Transfer, please ensure that you provide MHCC access to view your USI transcript.

If your qualification/s are from before 2015, you will need to provide certified copies of your qualification and transcript.

Supporting Documents

A certified copy of your photo identification is required with your application.

Examples of who can certify your identification include:

  • Justice of the Peace
  • Pharmacists
  • Nurses
  • Police Officers
  • Post Office staff

Work Placement Supervisor Endorsement

As part of your enrolment, you are required to complete 80 hours of work placement. To ensure the successful completion of work-based assessments, we require your supervisor to complete and sign the Supervisor Endorsement Form.

Please ask your work placement supervisor to read and complete this form. This section is part of the Selection Criteria and must be agreed to and signed.
